Influences On Normal Physical Physical growth in early childhood is partially easy to measure and gives an idea of how children normally develop during this period. The average child in North America is less than three feet tall at two years of age. Physical growth contains no discrete stages, plateaus, or qualitative changes. Large differences may develop between individual children and among groups of children. Sometimes these differences affect the psychological development of young children. These differences create a nice variety among children. Most dimensions of growth are influenced by the child's genetic background. Also, races and ethnic backgrounds around the world differ in growth patterns. Nutrition can affect growth, but it does not override genetic factors. One factor in the cause of slow growth is malnutrition. Malnutrition can start as early as pregnancy. Low birth weight babies have an increased risk of infection and death during the first few weeks of life. Food-deprived children carry a greater risk of neurological deficiencies that result in poor vision, impaired educational attainment, and cerebral problems. Such children are also more prone to diseases such as malaria, respiratory tract infections or pneumonia. The illnesses of malnourished children can cause more lasting damage than in a healthy child. The destructive conjunction between low food intake and disease is magnified at the level of the hungry child. There is evidence, according to The Journal of Nutrition, that an estimated 50 percent of disease-related mortality among infants could be avoided if infant malnutrition were eradicated. It has also been shown that low birth- weight is associated with increased prevalence of diseases such as stroke, heart disease and diabetes in adult life. Most damage during the first few years of life cannot easily be undone. There are many reasons why some children never reach normal height. Some causes of short stature are well understood and can be corrected, but most are subjects of ongoing research. Achondroplasia is the most common growth defect in which abnormal body proportions are present. Achondroplasia is a genetic disorder of bone growth. It affects about one in every 26,000 births. It occurs in all races and in both sexes. It is one of the oldest recorded birth defects found as far back as Egyptian art. A child with achondroplasia has a relatively normal torso but short arms and legs. People sometimes think the child is mentally retarded because they are slow to sit, stand, and walk alone. In most cases, however, a child with achondroplasia has normal intelligence. Children with achondroplasia occasionally die suddenly in infancy or early childhood. These deaths usually occur during sleep and are thought to result from compression of the upper end of the spinal cord, which can interfere with breathing. This disease is caused by an abnormal gene. The discovery of the gene allowed the development of highly accurate prenatal tests that can diagnose or rule out achondroplasia. There is currently no way to normalize skeletal development of children with achondroplasia, so there is no cure. Growth hormone treatments, which increase height in some forms of short stature, do not substantially increase the height of children with achondroplasia. There is no way to prevent the majority of cases of achondroplasia, since these births result from totally unexpected gene mutations in unaffected parents. One treatment available for children is known as growth hormone therapy. The policy governing the use of growth hormone (GH) therapy has shifted from treating only those children with classic growth hormone deficiency to treating short children to improve their psycho social functioning. This has caused quite a controversy. Parents have described shorter boys as less socially competent and having more behavioral problems than that of the normal sample. Shorter boys describe themselves as less socially active but not having more behavioral problems than that of the normal group. This is according to a study conducted by the Children's Hospital of Buffalo and the State University of New York at Buffalo. The researchers conclude growth hormone therapy should not be administered routinely to all short children for the purpose of improving their psychological health. They urge that physicians consider both a child's short stature and psycho social functioning before making a referral for growth hormone therapy. Another factor in the growth of children is their change of appetite. Young preschoolers may eat less than they did as a toddler. This is also when they will become more selective and choosy with the foods they eat. These changes are normal and result from the slowing down of growth after infancy. Insect Courtship Rituals and Mating Habits Ah, romance. Because insects are so numerous, a good deal of work goes into finding a suitable mate. Females can be fickle, with such a wealth of insect bachelors from which to choose. If a male stands a chance at passing on his genes, hes got to do something to stand out in the crowd. Courtship rituals in insect mating include serenades, dances, nuptial gifts, physical touch, and even aphrodisiacs. Serenades Courtship songs differ from calling songs, which are broadcast from a distance to help females find the males. Crickets use distinct calling and courtship songs, for example. Once the female cricket is nearby, the male suitor sings his best courtship song to sweep her off her six feet. Fruit flies have no calling song but they do sing when a mate is in close range. The fruit fly male vibrates his wings in a pulsing, rhythmic pattern. His song lets the female know he is of the same species, and available to mate. Mosquitoes sing harmonic duets with each other, adjusting the frequencies of their songs simultaneously as they near the moment of copulation. Dances and Foreplay Any woman is a sucker for a man that can dance. Some male insects and spiders cha cha cha their way to love, performing elaborate dances for their chosen mates. Jumping spiders are famous for their ballroom skills. They can perform a linear dance, a zigzag dance, and even a sort of can-can with their forelegs. Certain male flies perform aerial dances around a female to attract her attention and win the right to mate with her. Some female insects like to be cuddled and caressed to get in the mood. This is especially true of the more primitive, wingless insects. Springtails, for instance, will touch each other with their antennae. Sperm transfer in apterygotes takes place externally, with the male depositing his sperm on a surface and then gently coaxing his partner to take it. Some dung beetles engage in a different kind of foreplay. Together, the pair rolls a ball of dung to serve as a nursery for their offspring. Nuptial Gifts Gift giving is another clever strategy employed by some male insects in their pursuit of a mate. Before seeking a partner, hangingfly males hunt and capture arthropod prey. They then lure a female closer using a chemical signal and offer her the food gift. She examines the prey, and if she finds the meal to her liking, they mate. If the gift is insufficient, she refuses his advance. Balloon flies take gift giving a step further by wrapping the prey in pretty, silken balloons. Females fly into a mating swarm of males and choose a partner, who presents her with his silk package. Dont give the males too much credit, though. Theyve actually learned to trick the females by offering them empty balloons. Some male insects, like Mormon crickets, produce a spermatophylax, a protein-rich wad which they attach to the females genitalia. The female eats the sperm-free offering, which may have cost the male a full 30% of his body weight. Thats a pretty substantial gift. Aphrodisiacs When all else fails, insects may try an aphrodisiac to make a partner willing to copulate. Male queen butterflies dust prospective mates with an aphrodisiac produced by hairpencils, brush-like appendages on the tip of the abdomen. If his magic dust works, she will fly to a nearby plant. He dusts her once more to be sure shes ready, and if she is, they consummate the marriage. On the other hand, insects sometimes employ anti-aphrodisiacs to turn away suitors. Certain ground beetle females produce methacrylic acid, a potent anti-aphrodisiac that not only repels males, which can knock them out for several hours. Male mealworm beetles apply anti-aphrodisiac pheromones to their female partners after mating, to make them less attractive to other males.

France and Poland especially received lands that had been important to the Germany economy. Germany also lost all of its worldwide outposts to various allied nations. The new country of Austria was carved from previously held German lands as was the former Czechoslovakia, now called the Czech Republic. The allies were given all of Germany’s mercantile marine ships, another severe blow to the German economy. On top of that, the Treaty required Germany to construct civilian and war ships for selected Allied countries. The Treaty also placed stringent restrictions on the Germany’s capacity to defend itself or to wage war. Germany was not permitted to possess heavy guns, tanks, armored cars, u -boats, Zepplins or airplanes, no air force of any type was allowed. The defeated nation could keep no more than one hundred thousand troops in its army and fifteen thousand sailors in its navy. Germany was forbidden by the Treaty to import materials used for war and was made to pay steep reparations to the Allied nations as well as to the territories it ceded. All types of valuables were seized to make these payments such as precious metals, building materials, vehicles and ships. Antibody Level After Hepatitis B Vaccination in Hemodialysis ABSTRACT OBJECTIVE The objective of this study was to determine the antibody level after Hepatitis B vaccination in chronic hemodialysis patients. METHOD: All patients undergoing chronic hemodialysis (HD) at the dialysis unit of Liaquat National Hospital, fulfilling the inclusion and exclusion criteria were enrolled between April 2013 and September 2013, after taking informed consent. AntiHbs (Hepatitis B surface antibody) titers were measured. Patients were differentiated as Immune and nonimmune based on antibody titers, with levels of >10 IU/l being considered as immune and levels of RESULTS: Out of 118 patients enrolled, 103 (87.3%) had an adequate antibody response and were considered immune while only 15 patients (12.7%) had an inadequate antibody response rendering them non immune. AntiHbs titers showed no significant co-relation with gender and duration of Hemodialysis therapy (p>0.05), while age was found to have significant correlation as younger age group ( CONCLUSION: Our study showed a very good Antibody response to Hepatitis B vaccination among hemodialysis patients that correlated with age with younger age group having a better response but no correlation to gender and duration of dialysis. KEYWORDS: Hepatitis B virus, Anti-HBs antibody, Hemodialysis, Prevalence, Vaccination. INTRODUCTION Hepatitis B virus (HBV) infection is a common but avoidable disease. Hepatitis B virus (HBV) is a DNA virus that can be communicated via saliva, body fluids, semen, vaginal fluids, blood products, sexual contacts or prenatally influencing 350-400 million persons round the globe (1-3). In contrast to general population, hemodialysis patients are at higher risk of acquiring Hepatitis B Virus because of direct exposure to blood products, shared hemodialysis devices, needle pricks and hemodialysis process which involve access to blood circulation.(4) Hence, Hemodialysis patients are vulnerable to infections with Hepatitis B Virus and hepatitis C virus (HCV). The prevalence of Hepatitis B Virus in hemodialysis (HD) patients varies significantly between countries, ranging from minimal in developed countries to very high in some developing countries. Despite the fact that many steps have been taken for the prevention of HBV infection like mass vaccination programs, implementation of thoroug h blood donor screening, awareness encouragement programs of erythropoietin use and generalize availability in hemodialysis centers, Hepatitis B Virus infection remains a major concern in Hemodialysis centers majorly in developing countries (5). Patients who are on maintenance hemodialysis are considered as high-risk group, resulting in high incidence and mortality. Therefore, to vaccinate them against the virus is mandatory. Compared to a response rate of over 90% in the normal population, only 50 to 60% of those with end-stage renal disease achieve adequate antibody levels following immunization (6, 7). Various tactics have been employed to overcome the low seroconversion rate like co-administering zinc, gamma-interferon, thymopentin, interleukin-2, and levamisole as immunostimulants or adjuvants as well as changing the injection mode (intradermal versus intramuscular) or doubling the vaccine dose (7, 8). Low immune response to hepatitis B vaccination in patients on HD is noticed in several studies but has never been studied in our population. Therefore our aim is to conduct a study in our population to determine the serum Anti-Hbs levels in these patients following vaccination. MATERIAL METHODS From April 2013 to September 2013, 118 patients undergoing HD in Liaquat National Hospital and Medical College were screened for anti-HBs. A questionnaire was used to collect the demographic data and duration of HD. All patients were included in HD unit who underwent primary vaccination within last one year (four doses: recombinant HB vaccine; 40 ug, i.m, at 0, 1, 2 and 6 months). Exclusion criteria included patients on immunosuppressive drugs, malignancy or HIV positive patients. Enzyme linked immunosorbent assay (ELISA,Biokit, Spain) was used to measure Anti-HBs antibodies titers. The data was analyzed by SPSS  ® for windows ® (version 14.0 Chicago, IL, USA). A p value RESULTS We enrolled total of 118 patients on Hemodialysis who were recently vaccinated. Demographics are shown in Figure 1.Patient’s age ranged from 20-71 years. 46.6 %( N=55) were Male with mean age 53.2  ±10.02 yrs and 53.1 %( N=63) were Females with mean Age of 51.59  ±10.63 yrs. Age was found to have significant impact on Hepatitis B surface antibody titer with patients 0.05). Out of 118 patients, 15 (12.7%) were found to have Inadequate response or Non-immune, where as, 104 (87.3%) had an Adequate response and responded well to the immunization. Duration on Hemodialysis ranges from 1-4 yr with mean duration of 1.97 ±0.77 years, most of the patients had less than 3 years of Hemodialysis 97.5% (N=115/118) and only 3 patients (2.5%) were in year 4. Duration of Hemodialysis failed to show any significant impact on Hepatitis B vaccination response rate (p>0.05). DISCUSSION: An increased risk of exposure to HBV infection is observed in patients on maintenance hemodialysis (9) It has been observed that after vaccination for Hepatitis B, hemodialysis patients develop lower antibody titers compared to healthy individuals, and even if they are immunized, their antibody titers falls shortly within a year(10). The present study showed a very high response to hepatitis-B vaccination among hemodialysis patients. One hundred and four (87.6%) patients showed good antibody response after vaccination. Previous studies in hemodialysis patients have shown a variable hepatitis-B vaccination response rate, ranging from 47%-73%.(11-13).Comparable good results to hepatitis-B vaccination in hemodialysis patients had also been observed in areas with intermediate endemicity (2-8%) prevalence of Hepatitis B Virus ,such as in Brazil , which approached 89.5% in one study.(14) A recent meta-analysis of 17 clinical trials showed decreased response to hepatitis-B vaccination among older dialysis patients(15) which might be attributed to age associated changes to immune status, where â€Å"older† was defined at age 50 yrs. Our patients mean age were 52.3 ±10.04 yrs correlating with Meta analysis age group, and our results are similar with older patients having less immune response.(11, 16, 17) In the present study, gender and duration of hemodialysis therapy did not have any correlation to hepatitis-B vaccination. These results are in agreement with those reported by Peces et al .(18).Dacko et al.(16) and Tele et al(14).Similarly, Roozbeh et al(19) also confirmed the same results and showed that gender did not differ between responders (immune) and non-responders (non-immune) to hepatitis-B vaccination. CONCLUSION: We report a very good response to hepatitis-B vaccination among hemodialysis patients that is neither co-relating with gender or duration of hemodialysis. This was a preliminary study in our population which only estimated the response rate against vaccination. Future studies are needed to determine the impact of nutrional status and adequacy of hemodialysis on the response rate of vaccination as previous studies has shown their influences over titer levels.

JUSTICE AND THE NATURE OF MORAL COMMUNITY A central principal of a just society is that every person has an equal right to "life, liberity, and happiness." Within that, an arguement for capital punishment forms along the following lines: some acts are so evil and so destructive of a community that they void the right of the perpetrator to life. A community founded on moral principals has specific requirements. The right to belong to a community is not unconditional. The privilege of living and pursuing the good life in society is not certain. The essential reason on which community is built requires each citizen to honor the rightful claims of others. The precious live in a moral community must be so highly honored that those who do not honor the life of others void their own right to membership. Those who violate the personhood of others, especially if this is done persistently as a habit must pay the ultimate price. This must be done for the sake of the community which was violated. We can debate whether some non-lethal alternative is a suitable substitute for the death penalty. But the standard of judgment is whether the punishment fits the crime and if it honors the nature of the moral community. LOVE AND AN IDEAL SPIRITUAL COMMUNITY Christian live, is unconditional. It does not depend on the worthiness or value of those to whom it is directed. It is persistent in seeking the good of others regardless of whether they return the favor or even deserve to be treated well on the basis of their own wrongdoing. An ideal community would be made up pf free and equal citizens devoted to a balance between individual needs and the advancement of common good. Communal life would be based on mutual love in which equality of giving and receiving was the social practise. Everyone would contribute to the best of ability. What would a community based on this kind of love do with those who committed brutal acts of terror, violence, and murder? It is a degree to which a product performance exceeds the consumers expectations. Prepurchase expectations are consumers beliefs about anticipated performance of the product. Postpurcha se perceptions are consumer’s thoughts about how well the product performed. Disconfirmation refers to difference between the two perceptions. 3 types of disconfirmation: 1. Positive disconfirmation occurs when product performance is better than expected.This lead to satisfaction or a pleasurable level of fulfilment. 2. Negative disconfirmation occurs when product performance is lower than expected. This thought lead to dissatisfaction. 3.
